Identify the person representing the firm. All fields are required.

This person will make sure all users abide by the terms and conditions for this service. As part of the verification process, Wellington City Council may contact this person to confirm that they submitted the form.

Section 3: Nominated Service Users
Partner to identify all staff / contractors who are to be authorised to obtain this service on behalf on their firm. At least one person must be nominated.
